At Camp Kanata we want to learn as much as we can about your child before he or she arrives. Having prior knowledge about a recent loss or major change in the child’s life, a learning difficulty, ADHD, medical or other behavioral characteristics makes a significant difference in helping us to be sensitive to your child’s needs for patience, understanding and reassurance – especially in the first few days of Camp. All information is kept confidential and is used for cabin assignments, to alert health staff to potential issues, and empower your child to have the most rewarding and fun-filled Camp experience possible.

You do not have to complete this process in one sitting. Information can be saved as you go, and you can return to any sections you skip when it is convenient for you.

Camper Forms Portal - Section Guidelines
Choose your Camper
Forms need to be completed for each child attending Camp.
Upload a profile photo of your child so we can recognize them on opening day. A photo is recommended, but not required.
We suggest a square, "yearbook" style photo.
Emergency Contacts and Pick Ups
Emergency Contacts and Authorized Pick ups are individuals other than a parent who are at least 21 years old. We require two contacts with at least one phone number each.
The Camper Information is an opportunity for you to share with us a brief description of your child, goals or concerns you have. This is where you will request a cabin mate. The deadline to request a cabin mate is May 17, 2019.
The Health Summary should be a review of current health conditions. For this section, you will need Insurance Policy information, your child's doctor and dentist contact information along with any special care and conditions.
Health History Form
The Health History Form is a printable document that requires an ink signature by a parent/ guardian. When complete, this form should be scanned and uploaded back to your child's record in the Upload Documents section of your portal.
Camper Physical Form
The Camper Physical is a printable document that should be completed and signed by your child's physician or doctor's office. Each camper is required to have a physical within one year of their first day at Camp. If your insurance carrier will not cover an annual physical until after May 1, please complete all other sections in your portal. In the Send to Camp section, you can request an extension by letting us know when your appointment is scheduled. Your child's physical must be uploaded to your portal within 72 hours of this appointment.
The American Camping Association requires us to maintain copies of your Health History Form, Camper Physical Form and Immunizations on file. These completed documents should be individually scanned as PDFs and uploaded to your child's record in this section.

When you have finished updating all sections above, you must complete the final Submission Form located in this section. This final submission will notify Camp that information in your portal has been finalized and is ready to be reviewed by our staff. If there are any issues with the forms you have submitted, the Camp Kanata staff will contact you directly.
