CAMPER PHYSICAL    

Each camper is required to have a physical within one year of their first day at Camp. No child will be allowed to remain at Camp without a completed Camper Physical (Health Examination) form.
 
If your insurance carrier will not cover an annual physical until after March 27, you should complete all other sections and select the Request an Extension option on the last step under the Forms Status and Submission section.
 
In your extension, you will be required to include the date for your child's doctor appointment. Your child’s signed Physical Form must be scanned and uploaded through your portal within 72 hours of this scheduled appointment.
 
To complete this form:

  ♦ A licensed physician must sign and date this form at the bottom.  
  ♦ Once signed, this form will need to be uploaded.
