If you have not taken the steps to prepare your documents, please return to the Documents Page.
 
You will not be able to finalize your forms unless each document in the list has been uploaded as an individual PDF file with the correct file name. 
 
PLEASE NOTE:
You SHOULD NOT attempt to use/ upload a copy of your child's Camper Physical from last summer; it will expire by the time your child attends Camp this summer. After scheduling an appointment with your child's provider, you can request an extension for this document after finishing all other sections of the portal.
