FORMS STATUS & SUBMISSION   
You must notify Camp that your forms are ready to be reviewed. Send your forms to Camp using one of the choices below.

Which option should I choose?   
Send to Camp
If you had no errors on the status page of each section, choose Send to Camp below.
 
Extension Request
If all sections were error free EXCEPT for your camper's Physical or Immunizations Form under Camper Documents, choose Request an Extension. You will need to provide the Clinic Name and scheduled date for your child's physical appointment.
 
Send an Update
If you have already sent your forms to Camp for review, your portal status will be listed as Under Review, Approved or Returned.  
If you have made a revision, you can choose to notify Camp that you have made a change using the Send An Update button.
