CAMPER DOCUMENT LIST   
Review the list of documents for your camper and file requirements below. You will not be able to finalize your forms unless each document in the list has been individually uploaded as outlined.
 
 
IMPORTANT FILE REQUIREMENTS   
File List: Each document listed below MUST be saved as an individual file.
Combining documents into one uploaded file will prevent you from being able to finalize your forms.
 
File Type: Documents MUST be in a PDF format; it should not be a photo of your form using a camera.
 
File Size: Each document should be 4MB (4000KB) or less in size.

File Name: Each document should be saved using the filename provided in the table below.
Using filenames that are different than indicated will delay the approval process for your child.
 
File Requirements: Please ensure each documents meets the requirements below below including signatures, dates, etc.
